Call claimed to be from Wisconsin Dept of health. I wouldn't take survey until I knew who it was from. She gave me a web site and the phone number 877-551-6138. I wasn't able to find the website with what she gave me, but later found it - one letter was off https://www.dhs.wisconsin.gov/stats/brfs.htm This seems to be a legit survey in conjunction with the federal CDC covering all 50 states. If I want to participate I have to call 877-551-6138 and set up an appointment
