Call claimed to be from Wisconsin Dept of health. I wouldn't take survey until I knew who it was from. She gave me a web site and the phone number 877-551-6138. I wasn't able to find the website with what she gave me, but later found it - one letter was off https://www.dhs.wisconsin.gov/stats/brfs.htm This seems to be a legit survey in conjunction with the federal CDC covering all 50 states. If I want to participate I have to call 877-551-6138 and set up an appointment.

They call every 1-2 days claiming they are not a telemarketer, but actually from the Delaware division of public health and to call this number back. Here’s the voicemail transcription: “Hello my name is I'm sorry that I missed you my name is Dave and I'm calling on behalf of the Delaware division _⁠_⁠_⁠_⁠_⁠_⁠_⁠_ _⁠_⁠_⁠_⁠_ to gather information about the health practices of people in Delaware this is not a telemarketing call your phone number was randomly chosen and will give you a call back soon in the meantime if you have any questions call us at 1-877-551-6138 thank you…”
